CCR: An Overview for CWS Social Workers

Description of Course: This course is designed to deepen the understanding of the impact of Continuum of Care Reform (CCR) on youth, families, and staff. Additionally, the course will provide education on the different levels of care, placement types for youth, and core services. Participants will gain insight on how CCR impacts families today and how assessment and thoughtful planning utilizing the Child & Family Team (CFT) are key to successfully transition a child/youth to a lower level of care. The course will use lecture, small group discussion, short video clips as well as application activities.
Intended Learning Objectives:
- Articulate the CCR goals and vision
- Identify the different placement options
- Understand the changes in placements types
- Connect teaming with SET values
- Identify who is on the Child and Family Team (CFT) and what their role is
- Understand the role of BHS and Probation
- Connect case plan assessment and needs to core services
- Identify Core Services
- Understand how Core Services differ for special populations
- Describe CCR to youth and families in youth and family-friendly terms
