Continuum of Care Reform

PCWTA is supporting the Southern Region Counties with the implementation of Continuum of Care Reform (AB403), with technical assistance and workforce development including coaching, in person training, and eLearning’s. This page will provide you with specific resources either developed by PCWTA or links to Statewide or National Resources that can assist child welfare agencies with their reform efforts.

Microlearnings

PCWTA has developed a 5-part series of microlearnings to help you understand the Continuum of Care Reform (CCR) implementation in California. These 3-5 minute sessions focus on the following topics:

  • CCR goals
  • Why teaming is important in CCR
  • Array of placement options options available to youth in CCR
  • CCR core services
  • Explaining CCR to Youth

You can view these microlearnings on your desktop computer, laptop or tablet (mobile phones are not recommended). Each microlearning will get you one step closer to understanding how CCR impacts you, your practice and the foster youth you serve.

Southern Counties CCR Regional Learning Collaborative: San Diego
March 23, 2016

It was a very informative session that focused on current county implementation plans for Continuum of Care Reform and Resource Family Approval. Notes from the county sharing have been posted below in the Resources section of this page.

In addition Ventura County was able to share some of their marketing materials for resource families in their community. These materials are also posted in the Resources section below for your convenience.

Southern Counties CCR Regional Learning Collaborative: Moreno Valley
May 3, 2017
This collaborative was held in Riverside County at the Moreno Valley Conference Center from 9:30 – 3:00. Counties brought teams of up to 15 to include Child Welfare, Probation, Behavioral Health, as well as county identified community stakeholders.
